We do not accept insurance. You have the right to inquire if your insurer offers out of network benefits for reimbursement and may submit your visit code and payment information. The amount reimbursed depends on the health plan out of network benefit if one is offered. We cannot determine whether your insurance will reimburse you for this service.
Submission of claims is entirely the responsibility of the payor/patient.
Consults/Initial Visits are face-to-face only and include up to 60 minutes of face to face time, a personal history and physical assessment, medication/record
review, treatment plan, safety assessment for areas of improvement needed in the care setting to help meet functional goals, strategies for improved mobility and function, written recommendations, prescriptions where appropriate, referrals for therapy with specific instructions and techniques prescribed by the physiatrist, or referrals to other specialists where appropriate.
Follow-up Clinic Visit — Up to 30 minutes face to face time. Follow-up visits
may be in-person or virtually

New visit procedure: Patient will need to have government issued photo identification card. New patient’s active billing credit card will be stored securely on file (only to be applied to outstanding FWP patient account balances or visit fees).
