Scope: Providers of Freedom Wellness Physiatry are not primary care
physicians and are not expected to act in such a role. We are physical
medicine & rehabilitation specialists who prescribe a rehabilitation
program, which may include medication when appropriate, to help
meet your rehabilitation needs. Clients are advised to consult their
PCP prior to starting our rehabilitation program. We are not liable for
expectations of guaranteed results. This is a physician directed team
approach of care to better serve you or your loved one in achieving
optimal outcomes. Your subscription or payment for services
acknowledges your agreement to this policy.

I am not your primary care physician. I will provide my expertise as a consultant in physical medicine & rehabilitation to help meet your rehabilitation needs. This may include directing your current home health team, prescriptions, house calls, OMM treatment, injections, nutrition/fitness recommendations, and referrals for other services.
